For patients and their loved ones dealing with illness, disability, treatment or a stay at either Lake View Hospital or Cottages, we offer a variety of social services. Our Case Management department assists with coordinating care for patients during their stay at Lake View, and after they’re sent home. Our Social Workers coordinate with physicians, nurses, patients and families to find the best care solutions. We also can put you in touch with financial, legal mental health and other community resources.

The Minnesota Board on Aging offers information and education to meet the needs of Minnesota’s older population, including a Health Care Directive form that you can adapt for yourself.
